No. The applicant is responsible for obtaining the vintage registration plate they wish to display on their antique or classic vehicle.
PennDOT recommends that applicants verify if the configuration of the vintage plate can be issued by checking the configuration on PennDOT's website. The customer can check the plate availability at www.dmv.state.pa.us by selecting the Online Driver and Vehicle Services link on the left-hand side navigation bar and click on the Personalized Vanity Plate Availability link under Vehicle Registration Services. By checking the plate availability, it does not approve your configuration. It is only used as a tool to identify whether another license plate with the same configuration is used.
No. These are two separate fees of $75 each. To obtain a parking placard, you will need to submit Form MV-145A ‘Person with Disability Parking Placard Application.’ This form will need to be certified by a Health Care Provider licensed in Pennsylvania or a contiguous state (New York, New Jersey, Delaware, Maryland, West Virginia or Ohio). A police officer can also certify the request but only when the applicant does not have full use of a leg or both legs, or is legally blind. NOTE: Health Care Providers may only certify disabilities within their scope of practice. To obtain a disability license plate, you will need to submit Form MV-145 ‘Application for Person with Disability, Hearing Impaired, Disabled Veteran, or Severely Disabled Veteran Registration Plate or Person with Disability or Severely Disabled Veteran Motorcycle Plate Decal' (Form MV-145). Like the 'Person with Disability Parking Placard Application' (Form MV-145A), this form will need to be completed by a Health Care Provider licensed in Pennsylvania or a contiguous state (New York, New Jersey, Delaware, Maryland, West Virginia or Ohio) or a police officer. NOTE: Health Care Providers may only certify disabilities within their scope of practice. Requests for duplicate registration credentials (license plate, renewal sticker, or registration (owner’s) card) can be made on the 'Application for Duplicate Registration Card, Replacement of Registration Plate, Renewal Sticker or Weight Class Sticker’ (Form MV-44). Notarization is required only if you are requesting a free replacement of a previously issued product, which was not received within a 90 day period from the date of issuance.
