Changing Your Name
How to Change Your Name on Your Driver's License, Photo ID Card, Learner's Permit
All of the following forms are in PDF format and require Adobe

If your name has changed due to marriage, divorce or another reason, you will need to complete one of the following forms:
Commercial Driver's License: DL-80CD, or if expiring in the next 6 months: DL-143CD to renew.
If your name has been changed by permission of the court, you will need to attach a Certified Copy of the Court Order. If you desire
to use any name other than 1) your birth name, 2) spouse's surname or 3) a name given by the court, you must provide a copy of a Social
Security Card (or records), together with copies of documents from two other sources with the desired name such as: Tax Records, Selective
Services Card, Voter Registration Card, Passport or any form of Photo ID issued by a governmental agency.
How to Change Your Name on Your Vehicle/Trailer Registration and Title
If your name has changed due to marriage, divorce or another reason, you will need to complete form
MV-41A.
Side A of the form should be completed if you are only requesting an updated vehicle/trailer registration card. Side B should be completed if you are requesting an updated vehicle/trailer title.
